Consultation Contract
The goal of our work together is to support you in your work with couples, specifically with your use of Discernment Counseling.
As with any professional relationship, the alliance between us is extremely important. To develop the most collaborative working relationship, it is necessary for us to agree on some guidelines for our work. Please review these guidelines and sign in the space provided if you agree to these guidelines. If you have any questions or concerns about this agreement, please wait to sign and we will discuss it as soon as possible.
It is important to understand that I am considered a consultant and not legally responsible or liable for your clients. If you are not yet licensed, I will ask you to provide me with the name and contact information of your clinical supervisor. I will reach out to that person to make sure my provision of consultation services will not interfere with your clinical supervision.
You, as the Consultee, agree that:
•You will uphold all professional and ethical guidelines of your nationalorganization.•You will handle all crises and legal reporting situations independent of ourconsultation relationship.
•It is understood that “self of the therapist” issues may arise during ourconsultation work. You understand that these discussions are welcome, but arenot in any way to be misconstrued as personal therapy.
•You carry and maintain malpractice insurance.
•You are responsible for the confidentiality of your clients’ private information. You can insure that confidentiality in any way you see fit, but I recommend that youomit all names and other identifying information.
•You understand and endorse the following statement of indemnification:
Consultee shall indemnify and hold harmless Ginny Wright, Ph.D. and Drs. Lapporte, Wright& Assoc., PC from any liabilities, costs, and expenses, including attorney’s fees, established by judgment of alternative dispute resolution award, to have arisen in whole or in part, out of any grossly negligent act or omission of the consultee. Ginny Wright, Ph.D. and Drs. Lapporte, Wright & Assoc., PC shall indemnify and hold harmless the consultee from any liabilities, costs, and expenses, including attorney’s fees, established by judgment of alternative dispute resolution award, to have arisen in whole or in part, out of any grossly negligent act or omission of Ginny Wright, Ph.D.
